What to Include in the Report

      Purpose for the test. 
    Test objectives. 
    Testers profile. 
    Usability testing methods. 
    List of tasks completed. 
    The kind of questions asked. 
    Description of the test environment and the equipment used. 
    Data that was collected.

Highlights

    What worked. 
    What didn't work. 
    Interesting findings. 
    Interesting suggestions. 
    Respondents quotes. 
    Respondents recommendations. 
    Conclusions drawn from results.

Outline of Changes

It is helpful to have a screen shot printed of the pages to be changed to help the client visualize these changes more effectively.

    The emphasis should be on the short-comings of the site and possible changes to overcome these shortcomings.  
    Make a list of the changes and additions and categorize them into short term and long term goals.
    Mock up the changes on the screen shot copy and include them in the report. 
    Get the client feedback and have them sign off on the revisions.

Reference Report

Although each testing situation and each site is unique, it is valuable to keep a detailed reference of your testing experiences to use as a guide and for idea generation for future testing situations. Make sure you share this report with your company so they all can learn from the experience.

    Tester descriptions 
    When and where the testing was done Who the facilitators of the testing were. 
    List of questions and responses. 
    List of suggestions and recommendations from testers.
